The challenge

Sound familiar?

In Uganda's hospitals and clinics, a patient's entire medical history often exists on a piece of paper that can be lost, damaged, or simply unavailable at the moment it is needed most.

01

A mother's antenatal history disappears when she changes clinic

If a patient moves from a rural clinic to a referral hospital for delivery, her ANC records may never arrive. The receiving clinician treats her as a new case — and the risk to mother and baby rises sharply.

02

Mother and infant records are never linked — creating dangerous gaps

The same infant may be registered three times across three facilities, with no connection to the mother's records. Immunisation schedules are missed. The paper trail is broken at every transition.

03

Searching for a paper file in an emergency costs minutes patients don't have

When a nurse spends four minutes searching for previous visit notes in a stack of manila folders, that is four minutes not spent on care. In a maternal ward, that is not an acceptable trade-off.

What it does

Four functions. One system.

Patient Identity

Each card holds a unique patient ID, photo, and QR code — readable by any authorised scanner in any connected facility. No more duplicate registrations.

Mother-Infant Linkage

The mother's card is permanently linked to the infant's card at birth. Either can be scanned to pull up the full shared health history.

ANC Visit Tracking

All 8 antenatal contacts (ANC1–ANC8) are recorded against the patient's card. Missed visits trigger an alert.

Immunisation Schedule

The child's vaccination schedule is linked to the card from birth. Overdue immunisations are flagged automatically.

In daily operation

ZeroLost records with the card

Compatible with Uganda's NITA-U health data standards. No internet required for local reads; cloud sync where connectivity permits. Built after visiting maternal wards and watching what paper records cost nurses and patients.

"Before the Wellness Card, finding a patient's antenatal records meant searching through a filing cabinet for ten minutes. Now we scan the card and the full history is on screen in seconds. It has changed the way our ward rounds work."— Ward In-Charge — maternal & child health unit
